Coexistent delusions and hallucinations are frequent. ![]() Delusional jealousy is a frequent problem in dementia. Convictions about the partner's infidelities may form the content of psychopathological phenomena, such as delusions. We searched the electronic databases for original research and review articles on Othello syndrome in demented patients using the search terms 'Othello syndrome, morbid jealousy, pathological jealousy, delusional disorders, dementia'. The purpose of this paper is to examine the phenomenon of pathological jealousy in people with dementia. At least 30% of cases in the literature show a neurological basis for their delusion of infidelity, although its biological basis is not fully understood. Othello syndrome is a psychotic disorder characterized by delusion of infidelity or jealousy it often occurs in the context of medical, psychiatric or neurological disorders. Jealousy is a complex emotion that most people have experienced at some time in life pathological jealousy refers primarily to an irrational state. It is a very disturbing symptom for patients and their partners, often underestimated by them, and should therefore be actively searched for. Conclusions: We believe that OS is a more common psychiatric side effect in PD patients on treatment with dopamine agonists than usually believed, particulary in those with early disease onset. In three patients atypical neuroleptics had to be added: clozapine in two and quetiapine in one patient. In two patients, the symptoms remained the same. Decrease of dopamine agonist led to clinical improvement in three patients (complete reduction of the symptoms in two, reduction of symptoms in one patients). The first patient was treated with pramipexole, apomorphine infusion and levodopa/carbidopa, the second with apomorphine infusion plus levodopa/carbidopa/entacapone, the third with pramipexole, the fourth and fifth with ropinirole.
